Form Description

These will be displayed when the user first opens the form.

Form Opens At (optional) Leave blank for the form to be open immediately.

Form Closes At (optional) Leave blank for the form to remain open indefinitely.

Message when closed by end date (optional) Shown if the form is past its "closes at" time. Does not apply to override links.

Time Zone Select the time zone for form open/close times. Defaults to your current time zone.

Theme Colour Select the primary theme colour for the form.

Submission Message Enter a message to display when the form is submitted.

Form Banner Upload a banner image to display at the top of the form.

After publish, parents receive this email when they submit the form (optional PDF attachment).

Send confirmation email on submission

The message is sent to the address entered in the form’s email field.
